How To Register With and Transfer To Our Clinic.

Are You A Private Patient or Public Patient Holding a Medical or GP Visit Card?

Your status as a Private or Public Patient holding a Medical or GP Visit Card is important.

If you are a Private Patient, you can skip to the Registration step.

If you are a Public Patient and have a Medical or GP Visit Card, ordinarily you are free to continue with the registration process and proceed to the Registration step. However, if you are a registered Medical or GP Visit Card patient of Dr. Noreen Barry in Lismore, please continue reading.

If you are currently registered with a clinic whose list is locked/frozen and wish to transfer to Tallow Family and Women’s Clinic, you will need to send a written request to the HSE Primary Care Coordinator responsible for your area requesting that your file be released. If the HSE agree to release your card, you may continue with the registration process and proceed to the Registration step.

What is a locked/frozen medical card?

In the event of a sole doctor retiring or leaving their post, a medical card registered with that doctor may be locked by the HSE to the clinic. This will ordinarily remain in place until that post is filled with a new permanent doctor. The card remains locked until the position is filled plus 3 months after the new doctor has taken up the post. During this time, impacted patients will be unable to transfer to another doctor for routine care. After this, your card will become unfrozen and you will be free to transfer to a clinic of your choosing.

At the time of writing, Medical Card or GP Visit Card patients of Dr. Rafique in Youghal and Dr. Noreen Barry in Lismore are impacted.

If you have a Medical or GP Visit Card and are currently registered with a clinic whose list is locked/frozen, you will need to send a written request to the HSE Primary Care Coordinator responsible for your area stating your exceptional circumstances and reasons why your card should be released. If the HSE agree to release your card, you may continue with the registration process and proceed to the Registration step.

Once your file has been released by the HSE you may move to the Registration step.

All patients are required to complete our Patient Registration Form. This is so that we have your key information including name, contact details, medical history, allergies, medications, surgical history and PPS number.

This information is then used to create your Patient Record which is then stored in our secure Patient Management System (PMS).

Where more than one person, such as a family, is registering with the clinic, one registration form per person or child is required.

For you to register with our clinic, you must first be accepted by one of our doctors. Please download and complete our Patient Registration Form and bring it with you on the day of your appointment. Alternatively, you may attend the clinic and a member of our team will provide you with a copy and be happy to assist you with any questions you may have.

Transfer Your Medical Records.

Once your Registration has been accepted, you will be asked to complete a Transfer of Medical Records Request Form. This is an important step as it will allow our team to request your Medical Records from your current doctor and will give our practitioners access to your complete medical history.

Any person 18 years or older, even as part of a family transfer, will need to sign the transfer request form individually.

A member of our team will be able to help you with this process and will submit the request on your behalf. You are free to attend the clinic while this step is completed in the background.

HSE Change of Doctor Form.

If you are a Public Patient, holding a Medical or GP Visit Card, once your Registration has been accepted by our clinic, you will need to complete an HSE Medical Card/GP Visit Card Change of Doctor Form.

This form will need to be signed and stamped by the accepting doctor. Once complete, the transfer will then be processed electronically by a member of our team and you will be an active patient of our clinic.

This is NOT required for Private Patients.
